Suggestions for a power connector

I'm looking for a connector to use in a modular preamp project. I need 5 connections as follows:



1) Ground

2) B+ 250VDC @ 50 mA

3+4) 6.3VAC @.6A

5) 5VDC @200 mA



Ideally, this would be a panel mount female and cable mount male pair.



Any suggestions?
